CoF additional site application

This form is used by a certificate of fitness (CoF) inspecting organisation (IO) applying for an additional site to be approved for CoF inspections.

Before completing this form you should read the Site requirements for warrant of fitness and certificate of fitness service delivery document on our website, or you can request a copy by emailing info@nzta.govt.nz or by calling us on 0800 699 000.

A person applying on this form must:

  • be an existing CoF inspecting organisation or have applied to become a CoF inspecting organisation
  • complete all fields in the application form.
  • pay the application fee.

Inspecting organisation declaration

I declare that the information supplied in this application is correct and that if appointed I will comply with the Rules and Regulations. I authorise the NZ Transport Agency to make all enquiries as to my character and suitability to operate an inspecting organisation for the purposes of this application and for the term of any appointment as an inspecting organisation (under the Official Information Act 1982 and the Privacy Act 1993). *

Fees

An additional site application is charged at an hourly rate of $184.00 (GST incl).

You will be invoiced within one month from the completion of the site visit. The invoice will be due on the 20th of the following month. Failure to pay this invoice may result in the suspension of your authority to operate at the site until full payment is made.

Your information

Collection of this information is authorised by the Land Transport Rule: Vehicle Standards Compliance 2002. All information contained in your application form will be held by the NZ Transport Agency and treated as confidential subject to the Official Information Act 1982 and the Privacy Act 1993.

Under the Privacy Act 1993, you have the right to request access to and correction of any personal information you supply as a part of this application process from the Transport Agency.
