This factsheet advises what documents are acceptable as proof of your identity, address and change of name. When you are applying for a licence, renewing your licence or when you want to change the name on your licence you need to provide evidence of your identity and address.

Factsheet orders are dispatched in 3 working days.
For all other types of publications, North Island orders are dispatched via overnight courier for delivery the next working day (if order received by 11am); South Island orders are dispatched via standby courier for delivery in 48 hours (if order received by 11am).
