Safety Management Systems (SMS) - theme audit of safety management system uptake

Safety Management Systems have been adopted in various forms by RCAs since the mid 1990s. In 2003, the NZ Transport Agency (as LTSA) developed a set of guidelines & a programme to assist RCAs to develop an SMS. Most RCAs now have an SMS in place. This theme audit of a sample of twenty RCAs was to ascertain how useful & effective the SMS has been in providing safety outcomes.
