To use Driver Check, you must be a registered user.

Get an application pack

Download an application pack and print it, or send us an email at if you need us to email you a copy.

If you email us, please let us know your company's:

  • name
  • incorporation number
  • mailing address.

What’s in the application pack

The application pack includes:

  • general information about Driver Check
  • an application form for your company
  • terms and conditions
  • an application form for your individual staff members who'd like to become approved administrators
  • a direct debit authority form
  • a consent form for your company's drivers.

Company information needed

As part of the application, you'll need to provide these details for your company:

  • company name
  • trading name (if applicable)
  • contact person
  • contact person's position
  • physical address
  • mailing address
  • phone number
  • email address.

You'll also need your company's Certificate of incorporation.

Sending in your application

You need to send us these completed forms from your application pack:

  • Application to use Driver Check
  • Application to become an approved administrator for Driver Check
  • Driver Check direct debit authority
  • Driver Check credit application.

You don't need to send us your drivers' consent forms, unless we request them later.
